Have you tried the National Bureau of Statistics (NBS)  http://nbs.go.tz/

The 2012 Census Questionnaires can be downloaded here:

http://www.nbs.go.tz/nbs/takwimu/census2012/Short_Questionairre.pdf

This will give you some idea of what questions were asked in relation to Disability.
Unfortunately I don't have all the answer codes.
